To see one of our specialists, have your primary care provider fax us or print a referral form for them to fill out. Don't have a primary care provider? Click here
Which patients will benefit most from a virtual consultation?
We will aim to accommodate all patients; however, some may require an in-office assessment or procedure, and these will be directed to an office follow-up or referred to an appropriate provider by one of our specialists.
Ensuring the patient has the following will enable a rapid consultation process:
